Fall Sports Physical Packet 2025-2026

If the student is getting a physical by their own family physician, please print out this form and take it with you.  We will also have copies in the High School Main Office.

Winter 2025–2026 Sports Physical Packet

If you did NOT participate in a Fall sport, this is the packet you need to complete—not the Recertification Packet.

You will also need to get a physical from your doctor. Alternatively, you can receive a physical at the high school on October 6.
